Responsibilities Major Responsibilities: -Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems. Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens based on patient’s size, organ functions, and disease states. -Facilitates medication-related continuity of care at admission, transfer and discharge. -Reviews and accurately interprets medication orders. -Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling. -- -Practices in compliance with department policies & procedures, as well as local, state and federal regulations. -Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice. -Actively supports procedures and initiatives to prevent medication events. Reports events through appropriate channels. Actively supports department medication safety and quality improvement activities. -Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. May be preceptor-in-training. Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
Requirements Licensure, Registration, and/or Certification Required: -Pharmacist license issued by the state in which the team member practices. Education Required: -Bachelor's Degree in Pharmacy. Experience Required: -No experience required. Knowledge, Skills & Abilities Required: -Maintains skills in area of practice. -Strong computer skills. -Excellent communication and organization skills. -Excellent presentation, facilitation and training development skills. -Demonstrated decision making, team building, customer service, and conflict resolution skills. Physical Requirements and Working Conditions: -Must be able to stand for long periods of time. -Will occasionally lift up to 35 lbs. without assistance. -Must have functional speech, vision, and hearing. -Will be exposed to chemical hazards. -Operates all equipment necessary to perform the job. -Exposed to normal office environment.
