However, you must still complete the check-out process to register. The cost for non-members to attend is $20. You may attend once as a non-member for free but thereafter must join ICHP as a member.

Learning Objectives:

At the conclusion of this knowledge-based continuing pharmacy education activity, pharmacist participants should be able to:

1. Define transitions of care.
2. List the intervention categories.
3. Describe the role of pharmacists in transitions of care.
4. Given a patient scenario, select an appropriate intervention for a patient transition of care.

      Attendees must complete a program evaluation and CPE submission online at www.CESally.com. After successful CPE submission to CPE Monitor, attendees may print their statements of continuing pharmacy education credit at NABP.pharmacy.

      Target Audience: This knowledge-based activity was developed for Health-System Pharmacists 

      PTCB requires CPhTs to submit 20 hours of pharmacy technician-specific (‘T-specific’) continuing pharmacy education (CPE) to qualify for recertification. Please advise all pharmacy technicians at your site of this change. As an ACPE accredited provider, ICHP will continue to include Technician specific programming in most if not all of our CPE programming.  For PTCB recertification, CPhTs may use up to 5 hours of P-specific credit hours and CPhT-Adv up to 10 hours of P-specific credit hours.

      CPE Monitor: When registering for this program, you will be asked to provide your NABP e-Provile ID # and Birthday. This information will be required by any participants seeking CPE credit.  Visit www.ichpnet.org/cpemonitor/ for information about CPE Monitor and how to obtain your NABP ID.
