Who? Members of ICHP practicing in a health-system setting who are finding creative solutions to either old or new professional challenges
When? Submit a completed application by 12am CST July 1st, 2013
Where? Applications are available on the ICHP website
How? Applicants will be judged on the following:
- Innovativeness/Originality
- Contributions to improving patient care
- Contribution to institution and/or pharmacy practice
- Scope of the project
- Quality of the submission
Why? Award recipient will receive state-wide recognition, a plaque, and a monetary award to their institution!
