Selection Criteria
The Illinois Health-System Pharmacist of the Year Award is given annually, if a recipient is chosen, to an individual of high moral character, good citizenship and high professional ideals, who has made significant contributions affecting the practice of health-system pharmacy throughout the state. These contributions shall have been in the form of sustained exemplary service in health-system pharmacy, or a single outstanding achievement, or a combination of accomplishments benefiting health-system pharmacy, and through it, humanity and the public health. They may include accomplishments, achievements or outstanding performance in:
- health-system pharmacy practice,
- health-system pharmacy education,
- health-system pharmacy administration,
- pharmaceutical research or development related to health-system pharmacy,
- pharmacy organizational activity with a definite relationship to health-system pharmacy,
- scientific or professional pharmaceutical writing, e.g., noteworthy articles on pharmaceutical subjects with applicability to health-system pharmacy,
- pharmaceutical journalism related to health-system pharmacy,
- public and/or interprofessional relations activities benefiting health-system pharmacy,
- pharmaceutical law or legislation, professional regulations, standards of professional conduct or pharmacy law enforcement as related to health-system pharmacy practice.

Award Criteria:
- The individual nominated to receive this award must be an ICHP pharmacist, associate or technician member in good standing;
- The individual should be an exemplary preceptor, professor and/or mentor of students, residents, pharmacy technicians and/or new practitioners;
- The individual should be a positive role model for pharmacists, pharmacy students and/or pharmacy technicians;
- In order to be considered for the award, individuals must have been nominated using the approved nomination form below;
- More than one person may complete a nomination form for an individual.
Award Process:
- Each spring, a call for nominations will appear in KeePosted.
- The nomination form will be available on the ICHP website: www.ichpnet.org/awards
- Nominations are due by July 1 of each year at the ICHP office.
- The ICHP Executive Committee will review all nominations and select the finalists.
- The ICHP Board of Directors will select a recipient, should one be chosen.
- The award winner will be notified prior to the ICHP Annual Meeting to ensure attendance, and the award will be presented during the awards banquet.
- In the event that there are no suitable nominees for the award, an award will not be granted.

Award Purpose:
The ICHP Pharmacy Technician of the Year Award is established to identify and recognize exceptional performance by a certified pharmacy technician within the State of Illinois.
Award Criteria:
The candidate must:
- Be a current ICHP technician member
- Be a PTCB certified pharmacy technician for at least two years
- Demonstrate at least one of the following:
- exceptional contributions to his/her pharmacy worksite
- exceptional contributions to ICHP as a volunteer member
- exceptional contributions to the practice of pharmacy in Illinois
Nomination Process:
A call for nominations will be made in the May KeePosted each year. That announcement may be augmented with appropriate e-mail blasts. Nominations are due by July 1.
The nominator must:
- Be the technician’s supervisor, colleague or co-worker. No self-nominations will be accepted.
- Provide the following information:
- Worksite name, address and telephone number.
- Technician name and year certified.
- Describe the technician’s contributions in detail.
- Provide the nomination to the ICHP office by no later than July 1.
Selection Process:
All nominations are reviewed by the Division of Marketing Affairs at their July conference call.
- The Division will select two finalists for consideration and present them to the ICHP Board of Directors at the July Board Meeting.
- The ICHP Board of Directors will select the award winner from the two finalists presented by the Division of Marketing Affairs.
- The Division of Marketing Affairs may recommend one of the two finalists by providing detailed discussion points to the Board of Directors.
- The Board is not required to select a recipient if no candidate seems qualified.
Award Presentation:
The Award will be presented at the ICHP Annual Meeting in September, if a technician is selected.
- The award recipient and her/his nominator will be notified immediately following his/her selection by the Board of Directors.
- The award recipient will receive a complimentary full registration to the ICHP Annual Meeting.
- The award recipient will receive a plaque to be presented at an appropriately agreed upon time during the ICHP Annual Meeting. The nominator will be invited to participate in the award ceremony.
- The award recipient may give a continuing education presentation at the technician session at the following year's ICHP Annual Meeting on a topic of their choice related to their area of practice.

Award Criteria:
The objective of the Best Practice Award program is to encourage the development of innovative or creative pharmacy practice programs or innovative approaches to existing pharmacy practice challenges in health systems within the state of Illinois.
Applicants must be a member of ICHP for a minimum of 90 days prior to the submission dadline and practice in a health system setting.
Applicants will be judged on their descriptions of programs and practices employed in their health system based on the following criteria:
* Innovativeness / originality
* Contribution to improving patient care
* Contribution to institution and pharmacy practice
* Scope of project
* Quality of submission
Nomination Process:
A call for nominations will be made in the Spring and in the May KeePosted each year. That announcement may be augmented with appropriate e-mail blasts. Nominations are due by July 1.
Best Practice Online Submission Form
Instructions for preparing manuscript
Each entry for the Best Practice Award must include a manuscript prepared as a Word document, double-spaced using Times New Roman 12-pitch type. A header with the paper title and page number should appear on each page.The manuscript should not exceed 2000 words in length (not counting references), plus no more than a total of 6 supplemental graphics (tables, graphs, pictures, etc.) that are relevant to the program. Each picture, graph, figure, and table should be mentioned in the text and prepared as a separate document clearly labeled.
The manuscript should be organized as a descriptive report using the following headings:
* Introduction, Purpose, and Goals of the program
* Description of the program
* Experience with and outcomes of the program
* Discussion of innovative aspects of programs and achievement of goals
* Conclusion
Format
Submissions will only be accepted via online submission form. The manuscript will be forwarded to a pre-defined set of reviewers. Please do not include the names of the authors or affiliations in the manuscript to preserve anonymity.
All applicants will be notified of their status within three weeks of the submission deadline. Should your program be chosen as the winner:
* The program will be featured at the ICHP Annual Meeting. You will need to prepare a poster to present your program and/or give a verbal presentation. Guidelines will be sent to the winner.
* You may be asked to electronically submit your manuscript to the ICHP KeePosted™ for publishing as a continuing pharmacy education home study program.
* You will receive a complimentary registration to the ICHP Annual Meeting, recognition at the meeting and a monetary award distributed to your institution.
Non-winning submissions may also be considered for publication in the ICHP KeePosted™, with advanced permission of the primary author. The submission may also be considered for presentation at a meeting or webinar.

Selection Criteria
Created in 2024 - The ICHP Diversity, Equity, Inclusion and Accessibility (DEIA) Impact Award recognizes an ICHP member who has shown exceptional dedication and impact in their efforts to promote or advance diversity, health equity, inclusion, and accessibility initiatives or programs via advocacy, education, clinical practice, research, or leadership within the pharmacy profession, healthcare institutions, or local communities.
Nomination Process:
Please submit a nomination statement which reflects their philosophy, commitment, and work to promote diversity, equity, inclusion and accessibility. The statement can include activities within academic and professional associations, non-profit, governmental, and/or private sector organizations. The statement should be no longer than one page.
Provide the nomination to the ICHP office by no later than July 1st.
Award presented at ICHP Annual Meeting in the fall.

Selection Criteria
The Illinois Council of Health-System Pharmacists establishes the New Practitioner Leadership Award to recognize an individual whose early accomplishments distinguish them as future leaders in the profession. The candidates should be individuals whose performance demonstrates a high degree of professionalism, leadership, and innovation. Nominations are due by July 1. Nominations may be submitted to the ICHP Board of Directors or ICHP staff by an affiliate chapter or any two individual members of the ICHP. If a nominee is selected to receive the award, it will be presented at the Annual Meeting. Nominees should meet the following criteria:
1. Earned his/her first pharmacy degree within the last 10 years
2. Encourage and support ICHP by membership, meeting attendance, and participation*
3. Demonstrate a consistently high level of professional achievement
4. Has developed, is working in or is working towards the development of an innovative practice in Health- System pharmacy practice
5. Exhibit strong leadership qualities
6. Possess a strong commitment to the advancement of the pharmacy profession
* Membership in ICHP and volunteer activity within ICHP’s divisions, committees or board of directors are absolute requirements to receive this award.

Selection Criteria
The Outstanding Volunteer Award recognizes a member of the Illinois Council of Health-System Pharmacists for contributions to the Council that are considered above and beyond the normal for a volunteer member. These contributions may be on behalf of the Council or pharmacy in general and may be made at the local, state or national level. The Executive Committee of ICHP has the sole responsibility of the selection of the recipient of the award and should complete the selection process approximately one month prior to the ICHP Annual Meeting. The Executive Committee may select any member regardless of membership category as the recipient of this award.
The award is presented by the President-elect at the ICHP Annual Meeting in an appropriate setting that will provide a maximum of recognition for the recipient and their contributions. The Executive Committee will notify the ICHP office staff of their decision so that the appropriate award may be procured. The Executive Vice President or other ICHP staff member shall also notify the recipient in advance to ensure that the recipient is present for the presentation ceremony unless it can be guaranteed that the recipient will be present through some other mechanism.
The Outstanding Volunteer Award recipient will also be recognized with all other ICHP award recipients in the issue of the KeePosted that immediately follows the Annual Meeting. A copy of the award presentation text will be provided to the recipient so that their achievement may also be recognized by their employer.

The ICHP Executive Committee selects the receipient of the Outstanding Volunteer Award.

Nominations may be submitted to the ICHP Board of Directors or ICHP staff by July 1 by an affiliate chapter or any two individual members of the ICHP. If a nominee is selected to receive the award, it will be presented at the Annual Meeting. Nominees should meet the following criteria:
1. The individual has made contributions to the practice of pharmacy in the state of Illinois beyond the scope of duty and responsibility.
2. The individual has been an effective liaison between Health-System pharmacy practitioners and the manufacturer enabling the manufacturer to respond to new developments in Health-System pharmacy practice.
3. The individual has encouraged and supported the ICHP over the years by membership, and meeting attendance and participation.
Selection of a recipient shall be by the ICHP Board of Directors in a secret ballot. The successful nominee must receive the support of at least 3/4 of the Council's Board members. Only one ballot shall be taken and all Board members must be polled.
The award is presented during the ICHP Annual Meeting.
Nominations are due to ICHP office by July 1.

Selection Criteria
Established in 2023, the Legislator of the Year award honors Illinois lawmakers for their efforts in support of advancing pharmacy practice and public health in Illinois.
Legislator of the Year, Illinois Senate
Nominees who meet the following criteria are considered for the award:
* Member of the Illinois Senate
* Promoted advances in pharmacy practice in or through government service
* Contributed greatly to the advancement of public health through elected and career government service
* Is an outstanding leader in his/her field
Legislator of the Year, Illinois House of Representatives
Nominees who meet the following criteria are considered for the award:
* Member of the Illinois House of Representatives
* Promoted advances in pharmacy practice in or through government service
* Contributed greatly to the advancement of public health through elected and career government service
* Is an outstanding leader in his/her field
The ICHP Division of Government Affairs has the sole responsibility of the recipient of the award and should complete the selection process prior to the ICHP Annual Meeting.

Amicus Pharmacia Award

Selection Criteria
Established in 2025, the Amicus Pharmacia (Latin for Friend of Pharmacy) award honors a citizen of Illinois for their contributions to the profession of pharmacy through efforts in support of advancing pharmacy practice and public health in Illinois.
Nominees who meet the following criteria are considered for the award:
* Have significantly contributed to the advancement of pharmacy practice through a role in private or government service
* Are a resident of Illinois
* Are an outstanding leader in their field
The Amicus Pharmacia Award is presented as merited, recognizing individuals whose contributions align with the award’s purpose. It is not required to be awarded annually.
The ICHP Division of Government Affairs is responsible for selecting the recipient of the award prior to the Annual Meeting.
Any ICHP member may nominate an individual for the award.
Provide the nomination to the ICHP office by no later than June 30th via the online nomination form.
The ICHP Division of Government Affairs should complete the selection process prior to the ICHP Annual Meeting. The award is presented during the ICHP Annual Meeting.

Selection Criteria
The selection criteria are divided into three categories, as outlined below.
1. Criteria documenting the presence of the student chapter at ICHP activities:
a. Percentage of student members in the chapter (% of pharmacy class)
b. Percentage of student members who attended annual or spring meetings
c. Percentage of student members who presented posters at the annual or spring statewide meetings
d. Percentage of student members who attended Legislative Day
e. Percentage of student members who attended Board of Director meetings (or sent in report)
f. Number of articles submitted to the student chapter column of KeePosted
g. Number of community activities promoted on the ICHP website and Facebook page
2. Criteria documenting the activities of the student chapter that promote the mission of ICHP:
a. Number of residency preparation activities (include a description of the program, names of speakers, dates, and number of student attendees)
b. Number of health-system pharmacy related speakers presenting at student chapter meetings during the year
c. Number of community activities performed by the student chapter (include a brief description of the event, location, dates, and number of student attendees)
3. Criteria that document how the activities promote the mission of ICHP:
a. An essay that describes how the chapter’s leadership and other specific activities during the year promote the mission of ICHP. Specific activities can include those listed above in section two or those that focus on leadership development, professional growth, advancing the practice of health-system pharmacy, improving patient care, advocacy, or professionalism. The essay should not exceed three pages in length, 10 point font, double-spaced.
ICHP staff review the criteria and evaluate the chapter submissions. A certificate is awarded to a representative of the Student Chapter at the ICHP Annual Meeting Awards Ceremony.
Completed nomination applications must be submitted by July 1 to the ICHP office.

Selection Criteria
- Must be an ICHP member in good standing who has shown increased initiative as an ICHP Champion or committee member over the past year.
- Candidates cannot win 2 years in a row.
- This award may be given to an unlimited number of members per year.
- Winners may be nominated by ICHP staff members and members of the ICHP Board of Directors.
- Winners may be on the Board of Directors, as long as they meet all of the other criteria for the award.
- Candidates cannot nominate themselves for the award.
- Nominations for the Shining Star Award will be solicited via verbal announcement at ICHP Board Meetings and via email reminders.
- Nominees will be reviewed by the Executive Committee and winners will be chosen via final vote from the ICHP Board of Directors.
Winners will be announced by name and asked to stand during the annual meeting awards banquet and receive a paper certificate. They will also receive name recognition in KeePosted following the meeting.
