The Illinois Council of Health-System Pharmacists (ICHP) supports the practice of therapeutic interchange.
Therapeutic equivalents are drug products differing in chemical composition or in their basic drug entity that are considered to have very similar pharmacologic and/or therapeutic actions. Therapeutic interchange occurs when therapeutic equivalents are used with prior prescriber input. Prescribers, in collaboration with pharmacists and through the Pharmacy and Therapeutic Committee (or its equivalent), should review available drugs and agree upon those that have equivalent therapeutic effects. These drugs are then mutually viewed as being therapeutically interchangeable. The process should ensure that prescribers are fully informed regarding drug therapy options and interchanges which may result. Prescribers retain ultimate authority to decide upon patient therapy.
