Aurora Health Care is the largest, not-for-profit health care system in Wisconsin.  It is a fully integrated health care system encompassing 15 hospitals, numerous ambulatory, diagnostic and treatment centers, home health care, social services agencies, physician clinics, retail pharmacies, and more.  Each resident’s program is customized to their interests and needs.

The PGY1 Pharmacy Residency Programs are structured to provide a broad base of skills, knowledge and experience to the entry-level practitioner.   These programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.  These hospital-based programs are offered at several facilities within Aurora Health Care - each with different sizes, locations, and specialties.

The PGY1 Community Pharmacy Residency Programs provide residents with the knowledge and skills necessary to practice as community pharmacists in a decisive and proactive manner, placing an emphasis on the provision of clinical services.  These programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.  Aurora Health Care offers these programs in several different settings:  hospital-based community, community-based, and within a specialty pharmacy.

The PGY1 & 2 Health-System Pharmacy Administration Residency Program is aimed at training the future leaders of integrated health systems. It is offered as either a one-year administration program for residents with prior residency experience or is combined with the PGY1 Pharmacy-Aurora Health Care Metro, Inc Residency for a two-year program for applicants without prior residency experience. 

The PGY1&2 and PGY2 Pharmacy Informatics Residency Programs prepare clinicians to practice as pharmacy informatics specialists and leaders within an integrated healthcare organization.  Training in this specialty is offered as either a one-year PGY2 program for residents with prior residency experience or is combined with the PGY1 Pharmacy-Aurora Health Care Metro, Inc Residency for a two-year program for applicants without prior residency experience. 

The PGY2 Critical Care, Oncology, Cardiology, and Infectious Diseases Pharmacy Residency Programs are structured to provide specialized skills, knowledge and experience in a variety of settings within the specialty area.  These programs prepare clinicians to practice as clinical specialists and practice leaders in their specialty area.

After completing this program, the resident will be prepared to seek a clinical pharmacist position in an acute care setting or a PGY2 resident position.

This 12-month post-graduate program offers the pharmacy resident comprehensive training in a variety of community and ambulatory settings. During the program, the resident will staff in both retail and ambulatory pharmacies. This 24-month post-graduate program prepares the resident to assume high-level managerial, supervisory and leadership responsibilities. Jesse Brown VA Medical Center (JBVAMC) is a 200-bed tertiary care facility classified as a Clinical Referral Level II Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology as well as education and research. Comprehensive health care is provided through primary and tertiary care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, and geriatrics. Clinical pharmacy services at Jesse Brown VA Medical Center are provided by pharmacists employed by the VA and the University of Illinois College of Pharmacy. Decentralized clinical pharmacists provide pharmaceutical care to the inpatient population. Ambulatory care involvement includes pharmacy-managed clinics, as well as participation in interdisciplinary primary care clinics with physicians, nurses and dietitians. Jesse Brown VA Medical Center has active affiliations with the Northwestern University Medical School and the University of Illinois at Chicago Colleges of Medicine and Pharmacy. Over 900 University residents, interns, and students are trained at JBVAMC each year. The pharmacy service trains over 120 students from the University of Illinois yearly.

The Jesse Brown VA Medical Center PGY-1 Pharmacy Residency Program targets core areas of pharmacy practice, with an emphasis on ambulatory care, and fosters an integrated inpatient/outpatient team approach to patient care. The program accommodates for individual experience and specific areas of interest. The resident is exposed to quality assurance and medication use evaluation activities, and participates in formulary management through the Pharmacy and Therapeutics Committee. The resident will participate in the training of Doctor of Pharmacy candidates and will have the opportunity to earn a teaching certificate from the University of Illinois College of Pharmacy. Each resident conducts a research project and presents the results at the Great Lakes Pharmacy Resident Conference.

Memorial Medical Center is a 500 bed teaching-community hospital that serves as a primary teaching site for the Southern Illinois University School of Medicine (SIU-SOM). Memorial also serves as a pharmacy teaching site for the Southern Illinois University Edwardsville School of Pharmacy (SIUE-SOP), Midwestern University and St. Louis College of Pharmacy. The department of Pharmacy utilizes an advanced robotic dispensing system which allows unit-based pharmacists to provide a wide variety of clinical pharmacy services.

Memorial Medical Center is a three time, designated Magnet hospital for nursing, a Bariatric Center of Excellence, a Kidney and Kidney-Pancreas Transplant Center, Regional Cancer Center, Level I Trauma Center, Regional Burn Center and Comprehensive Stroke Center. Memorial is a clinical practice site for SIUE School of Pharmacy faculty, with practices in internal medicine and infectious diseases. Pharmacists at MMC participate in many hospital committees and provide education to physicians and other healthcare professionals.

The resident will also participate in hospital committees, complete a residency project, present at a regional conference and provide continuing education for practicing pharmacists. Additionally, residents will precept students.

The Pharmacy Practice Residency (PGY-1) at Mercy Health Saint Mary’s provides residents with a broad-based and customizable post-graduate clinical training experience and accepts two residents annually (with plans to increase to 4 residents beginning those starting July 2017).  MHSM department of pharmacy has dedicated, full-time, clinical pharmacists practicing throughout our health system including:  critical care, internal medicine, family medicine, emergency medicine, psychiatry, infectious diseases and antimicrobial stewardship, oncology, neurology, anticoagulation management service, ambulatory care (PCMH), renal transplant, and HIV medicine.  The required rotations, together with the large number of available elective rotations, including a Teaching and Learning Certificate Program and an elective rotation in Academia, allow residents to acquire experience and competence in a wide-range of pharmacotherapeutic areas so as to produce excellent clinical pharmacy practitioners.  Completion of the Pharmacy Practice Residency at Mercy Health Saint Mary's prepares pharmacists to provide well-rounded, direct patient-care within a health-system, to function as a clinical or adjunct faculty member, to pursue specialized training through the completion of a specialty (PGY-2) residency, and to be eligible for board certification as a pharmacotherapy specialist.

The CPS/MHMC residency is a structured, post-graduate experiential training program that strives to develop proficient and professionally committed practitioners. The residency will be customized to the residents’ interests and allows for significant flexibility. Required learning experiences include internal medicine, critical care, infectious diseases, ambulatory care, emergency medicine, drug information, and administration. Electives include cardiology, medication safety/informatics, teaching and advanced rotations. Residents will gain experience providing a wide range of pharmacotherapy, educational, and medication management services to patients, providers, and staff. In this program, the resident will actively participate in efforts to improve the scope and quality of pharmacy services, and will evaluate the relevant outcomes of such initiatives. As a clinician, the resident will demonstrate proficiency and responsibility in caring for patients with a wide variety of disease states in different healthcare settings. Residents will improve their abilities in clinical and operational pharmacy services and develop leadership skills that can be applied in any practice setting. Palos Community Hospital PGY-1 Residency Experience
The pharmacy residency program at Palos Community Hospital is designed to build on the didactic foundation learned and enhance the clinical pharmacotherapeutic skills taught during pharmacy school. The clinical practice experiences, operational experiences and health care administrative responsibilities afforded during the program sharpen residents’ critical thinking and problem-solving skills, aid in development of leadership skills that can translate into any practice area and allow opportunity self-reflection to strengthen professional values and opinions. Successful graduates of this program are well equipped to go on and function as members of multi-disciplinary teams to provide patient-centered care to adult patients with a variety of disease states and comorbidities.

Each learning experience is a standard rotation (4-6 weeks duration), a longitudinal experience (> 12 weeks) or a concentrated experience (<2 weeks). The HSHS St. Elizabeth’s Hospital Pharmacy Residency Program is affiliated with the Southern Illinois University Edwardsville (SIUE) School of Pharmacy and offers two PGY-1 positions. The majority of the resident’s time will be spent at Frankfort Regional Medical Center located in the picturesque capital city of Kentucky. SUCOP at Passport Health Plan in Louisville, KY:
Passport Health Plan and the Sullivan University College of Pharmacy have partnered to deliver a postgraduate training opportunity for pharmacists to expand their skill set in the area of managed care pharmacy. The post graduate year one (PGY1) residency program at Passport Health Plan is an educational program that will allow post graduate pharmacists to gain experience in Medicaid managed care pharmacy, including the areas of formulary management, prior authorization, strategic benefit design, and pharmacy operations; and including participation in the Pharmacy and Therapeutics (P&T) Advisory Committee. The PGY-1 residency at UCM is a directed, post-graduate training experience in general pharmacotherapy. Oncology: This specialty residency in oncology pharmacotherapy gives an in-depth experience in medical oncology, hematology (leukemia), stem cell transplantation, pediatric oncology, gynecology oncology, neuro-oncology and oncology ambulatory clinics. In addition, the resident will have learning opportunities in palliative care, immunocompromised infectious diseases, medical writing and benign hematology. Residents will train with the adult general and immunocompromised infectious diseases consult services, the UCM antimicrobial stewardship program, and in an outpatient infectious diseases clinic. In addition, learning opportunities are available in pediatric infectious diseases, leukemia, hematopoietic stem cell transplantation, the medical intensive care unit, infection control, and clinical microbiology. Residents have opportunities to present formal lectures at affiliated schools of pharmacy and may serve as preceptor for pharmacy clerkship students. The University of Minnesota PGY-1 Pharmacy Program specializing in Ambulatory Care is a multi-site program focusing on the practice of pharmaceutical care in ambulatory care settings organized around three emphasis areas throughout Minnesota:

Our span of sites is among the largest of any program nationally, offering residents a wide range of settings to become skilled practitioners through individualized learning experiences. Residents have opportunities to experience both rural and urban practices, within teaching focused clinics and non-teaching clinics, didactic and experiential teaching, and underserved and for-profit clinic settings.

Program Vision
Advancing ambulatory pharmacy practice in diverse communities by developing patient-centered leaders while providing comprehensive medication management in a healthcare team. Our residency graduates will combine expertise in pharmacotherapy with an interprofessional team approach to patient-centered care, assume responsibility for safe and effective medication use, successfully and efficiently manage practice, and demonstrate an enthusiastic commitment to leading practice in an evolving health environment. Our graduates have been immersed in an innovative/ cutting edge health care environment, and will be health care leaders that integrate and advance comprehensive medication management into practice. The Ambulatory Care Residency Program will be recognized nationally not only for its exceptional graduates, but also for our innovative approach to simultaneously meet the growing demand for residency opportunities while enhancing quality pharmacy services in health care organizations.

The University of Wisconsin Community Pharmacy Residency Program (UWCPRP) was developed in 2004 by a group of innovative community pharmacies involved in training community residents and the University of Wisconsin School of Pharmacy.  This multi-site program was created to forward innovative community pharmacy practice through the development of quality residency training programs designed to enhance the practice skills of today’s community pharmacist.  The UWCPRP represents a variety of community practice sites that offer innovative practice experiences and unique opportunities for teaching and research with faculty members at the School of Pharmacy.  The sites include:

VA Center for Medication Safety (VAMedSAFE), Hines, IL

PGY2 Medication-Use Safety Residency

The purpose of the PGY2 Medication-Use Safety Residency (1 position) at the VA Center for Medication Safety (VAMedSAFE) in Hines, Illinois is to prepare pharmacists with proficiencies in the planning, implementation, and assessment of safe medication use practices within an integrated health care delivery system, managed care setting, and/or drug safety oversight group. Residents graduating from this program will be qualified for any of the following positions:

• Pharmacy medication safety officer within a hospital or managed care organization;
• Pharmacy leader responsible for monitoring and resolving ADEs, as well as providing targeted education, in a health system, managed care area, or drug safety oversight agency (e.g., ISMP, FDA, CDC, etc.);
• Drug use management pharmacist;
• Study researcher in an industry setting, and
• Medication use safety or research faculty in academia.

VAMedSAFE is a comprehensive pharmacovigilance program that has developed and implemented numerous national programs to monitor the safe and effective use of medications in the VA Health Care System. VAMedSAFE has received national recognition for its efforts in drug safety and as a result, collaborates closely with the FDA to evaluate the significance of known adverse drug reactions and to identify and track new and emerging drug safety issues. As such, VAMedSAFE conducts drug safety and monitoring efforts with the rigor of a regulatory body while maintaining the access and utilization required for a health care system.

Wheaton Franciscan is a community hospital system providing acute and subacute services in Racine, Wisconsin (All Saints) and Milwaukee, Wisconsin (St. Francis and St. Joseph).

The pharmacy residency programs are designed to develop self-directed practitioners who are prepared to assume advanced practice in an acute care environment, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training. The resident will acquire or enhance current pharmaceutical care, leadership and administrative skills. Residents will function as effective members of the health care team with the ultimate goal of providing optimal pharmaceutical care to patients. Residents will demonstrate time management and independent thinking skills that will make them leaders within their practice settings and within the profession. Required rotations vary by program with all having Internal Medicine, Critical Care, Infectious Diseases, and Ambulatory Care rotations. Please see program website for more details.
