Aurora Health Care is the largest, not-for-profit health care system in Wisconsin.  It is a fully integrated health care system encompassing 15 hospitals, numerous ambulatory, diagnostic and treatment centers, home health care, social services agencies, physician clinics, retail pharmacies, and more.

The PGY1 Pharmacy Residency Programs are structured to provide a broad base of skills, knowledge and experience to the entry-level practitioner.   These programs build on Doctor of Pharmacy (Pharm.D.) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.

The PGY1 Community Pharmacy Residency Program provides residents with the knowledge and skills necessary to practice community pharmacy in a decisive and proactive manner, placing an emphasis on the provision of clinical services.  Residents completing this program will be change-agents and skilled patient care providers in community pharmacy settings.

The PGY1 & 2 Health-System Pharmacy Administration Residency Program is aimed at training the future leaders of integrated health systems. It is offered as either a one-year administration program for residents with prior residency experience or is combined with Aurora’s PGY1-Pharmacy-Metro, Inc Residency for a two-year program for applicants without prior residency experience. 

The PGY2 Critical Care, Oncology, and Cardiology Pharmacy Residency Programs are structured to provide specialized skills, knowledge and experience in a variety of settings within the specialty area.  These programs prepare clinicians to practice as clinical specialists and practice leaders in their specialty area.

The PGY2 Pharmacy Informatics Residency Program prepares clinicians to practice as pharmacy informatics specialists and leaders within an integrated healthcare organization.


Borgess Medical Center, Kalamazoo, MI

Borgess Medical Center is located in Kalamazoo, Michigan in the southwest region of the state. The purpose of the Borgess post graduate year one (PGY1) residency program is to create well-rounded, competent clinical practitioners of pharmaceutical care. The program integrates residents into the medication use process so they develop a firm understanding of the clinical, distributive, and administrative aspects of the pharmacy profession. It prepares them to be accountable and responsible for achieving optimal drug therapy outcomes as members of the health-care team. The program seeks to expand each resident’s presentation, teaching, evaluation, and research skills. Finally, the program strives to instill the importance of being engaged professionals, who are committed to the ongoing expansion of their expertise and to the advancement of the profession. Upon completion of the program, residents will be equipped with the knowledge and skills necessary to succeed in an advanced (PGY2) residency or fellowship program or to function independently as a pharmacist in an acute care setting.

The PGY1 Pharmacy Residency Program at Borgess accepts two residents each year and offers the following unique strengths:
  • As a 422-bed community, teaching hospital, we are large enough to provide ample educational opportunities, but small enough so you do not feel like number
  • All of our residency preceptors have graduated from an ASHP-accredited residency program, over half have also obtained advanced degrees (Pharmacy Administration) or specialty residency training
  • We offer residents the opportunity to participate in Ferris State University College of Pharmacy’s Pharmacy Education and Lecture Series (PEDALS) program

A research project is required.

Community Healthcare System is an integrated health care system consisting of St. Mary, St. Catherine, & Community hospitals; The Cancer Research Institute; and Community Care Network. An integrated, electronic medical record system with physician order entry is used throughout the organization. Community Hospital PGY-1 Pharmacy Practice Residency Program provides broad clinical training in a variety of areas. Service Commitment:
Establishing competency to function independently as a clinical pharmacist is an important component of the residency. While staffing the inpatient pharmacy every other weekend and on staffing weeks, the resident will review the appropriateness of therapy and understand the required monitoring for the various medications. The resident will be expected to be observant, think like a practicing clinical staff pharmacist and use the information they gain from direct experience of day-to-day practice to make practice decisions.

This PGY-1 pharmacy residency is sponsored by Comprehensive Pharmacy Services and is conducted at Mercy Hospital and Medical Center. The residency will be customized to the resident's interests, allowing significant flexibility. Required rotations include internal medicine, critical care, infectious disease, ambulatory care, drug information, emergency medicine, and administration. Longitudinal rotations include ambulatory care, staffing, and project management. Electives include cardiology, medication safety/informatics, and advanced rotations. As a clinician, the resident will demonstrate proficiency and responsibility in caring for patients with a variety of disease states. Throughout all experiences, the resident will acquire the skills necessary for lifelong learning and self-assessment.

This PGY-2 pharmacy residency in Ambulatory Care is sponsored by Comprehensive Pharmacy Services and is conducted at Mercy Hospital and Medical Center.  The residency is designed to transition PGY1 residency graduates from general practice to specialized practice to serve the needs of an ambulatory patient.  Required rotations include ambulatory care pharmacy practice I/II/III, primary care, health systems administration, ambulatory care specialty medicine, ambulatory care student precepting, emergency medicine, and transitions of care.  Longitudinal community pharmacy medication therapy management experiences take place throughout the year.  Electives include drug information, oncology infusion clinic, and ambulatory care medication safety. The resident helps advance the clinical goals of the ambulatory care pharmacy practice site and learns from the experience of his/her peers and other health care professionals.

The PGY-1 residency at FSMH is a comprehensive one-year program designed to provide individualized development while providing professional training and upon completion the resident will be prepared to practice as a clinical pharmacist or pursue a PGY-2 in the area of his/her choice.

  • The PGY-1 residency offers a variety of learning experiences and longitudinal requirements built to emphasize clinical practice which include; internal medicine, nutritional support, infectious disease, critical care, emergency medicine, informatics, management, anticoagulation clinic, drug information, regulatory compliance. Froedtert & the Medical College of Wisconsin, Froedtert Hospital;  is a 500 bed academic medical center and Level 1 trauma center located in Milwaukee, Wisconsin.  Pharmacists at Froedtert provide comprehensive clinical services and patient education in both inpatient and ambulatory settings. In addition to the PGY1 level pharmacy residency programs listed below, Froedtert offers PGY2 residency programs in critical care, oncology, drug information, ambulatory care, infectious disease and emergency medicine. Numerous teaching and precepting opportunities are available to residents as a part of all programs as well as an opportunity to participate in a teaching certificate program through Concordia University of Wisconsin.

PGY1 Pharmacy Residency Program
This 12-month post-graduate program offers the pharmacy resident comprehensive training in acute care pharmacy practice with elective opportunities in ambulatory care. After completing this program, the resident will be prepared to seek a clinical pharmacist position in an acute care setting or a PGY2 resident position.

This 12-month post-graduate program offers the pharmacy resident comprehensive training in a variety of community and ambulatory settings. During the program, the resident will staff in both retail and ambulatory pharmacies. After completing this program, the resident will be prepared to seek a clinical pharmacist position in a community or ambulatory settings or a PGY2 resident position.

Jesse Brown VA Medical Center (JBVAMC) is a 200-bed tertiary care facility classified as a Clinical Referral Level II Facility. It is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology as well as education and research. Comprehensive health care is provided through primary and tertiary care in the areas of medicine, surgery, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, and geriatrics. Clinical pharmacy services at Jesse Brown VA Medical Center are provided by pharmacists employed by the VA and the University of Illinois College of Pharmacy. Decentralized clinical pharmacists provide pharmaceutical care to the inpatient population. Ambulatory care involvement includes pharmacy-managed clinics, as well as participation in interdisciplinary primary care clinics with physicians, nurses and dietitians.  Jesse Brown VA Medical Center has active affiliations with the Northwestern University Medical School and the University of Illinois at Chicago Colleges of Medicine and Pharmacy. Over 900 University residents, interns, and students are trained at JBVAMC each year. The pharmacy service trains over 120 students from the University of Illinois yearly.

The Jesse Brown VA Medical Center PGY-1 Pharmacy Residency Program targets core areas of pharmacy practice, with an emphasis on ambulatory care, and fosters an integrated inpatient/outpatient team approach to patient care. The program accommodates for individual experience and specific areas of interest. The resident is exposed to quality assurance and medication use evaluation activities, and participates in formulary management through the Pharmacy and Therapeutics Committee. The resident will participate in the training of Doctor of Pharmacy candidates and will have the opportunity to earn a teaching certificate from the University of Illinois College of Pharmacy. Each resident conducts a research project and presents the results at the Great Lakes Pharmacy Resident Conference.

Loyola University Medical Center, Maywood, IL

Loyola University Health System, which is a member of Trinity Health, is an 801-bed multi-campus organization which includes the Loyola University Medical Center campus in Maywood (570 beds including the Cardinal Bernadin Cancer Center, the Ronald McDonald Children’s Hospital of Loyola, the Burn/Trauma Center, and the Center for Heart & Vascular Medicine) and the Gottlieb Memorial Hospital campus in Melrose Park.  Loyola offers a wide range of medical, surgical and obstetrical services including many specialty services.  Loyola University Medical Center is the only academic teaching hospital in the western suburbs of Chicago and is also a large referral center for this area.  Loyola is an accredited Level I Trauma Center, a Burn Center, and is served by an aeromedical program that transports patients from up to 150 miles away to Loyola.  Specialty services include transplantation, cardiovascular surgery, oncology, women’s health, primary care, and pediatrics among many others.  Loyola University Medical Center houses nine discrete critical care units (neurological, medical, cardiovascular, cardiac, and surgical/trauma, bone marrow transplant, burn, pediatric, and neonatal).Loyola University Health System offers a one-year post-graduate year one (PGY1) pharmacy practice residency.  The program is designed to offer a wide variety of clinical practice experiences in critical care medicine and surgery, general medicine, cardiology, pediatrics, transplantation, and infectious diseases. Graduates of this program will be well-rounded practitioners with the skills to practice in a variety of care settings. Graduates will possess the knowledge and critical thinking skills to function as members of a multidisciplinary team to provide pharmaceutical care to patients with a wide variety of disease states. Pharmacy residents are provided the opportunity to accelerate their growth beyond entry-level professional competence in patient-centered care and in pharmacy operational services, and to further the development of leadership skills that can be applied in any position and in any practice setting. Pharmacy residents acquire knowledge required for skillful problem-solving, refine their problem-solving strategies, strengthen their professional values and attitudes, and advance the growth of their clinical judgment. The core pharmacy residency experiences include inpatient care rotations in general medicine, pediatrics, critical care (2 rotations), infectious diseases, cardiology, administration, operations (longitudinal), and research. The pharmacy resident will complete 12 four-week rotations consisting of 9 required rotations and 3 electives.

PGY-1 Pharmacy Residency Program

General Description of Memorial Medical Center and Pharmacy Services:
 
Memorial Medical Center is a 521 bed teaching community hospital that serves as a primary teaching site for the Southern Illinois University School of Medicine (SIU-SOM). Memorial also serves as  a pharmacy teaching site for the Southern Illinois University Edwardsville School of Pharmacy (SIUE-SOP). The Department of Pharmacy utilizes an advanced robotic dispensing system which allows pharmacists to provide a wide variety of clinical pharmacy services.

Memorial Medical Center is a designated Magnet hospital for nursing, a Bariatric Center of Excellence, a Kidney and Kidney-Pancreas Transplant Center, Regional Cancer Center, Regional Level I Trauma Center, and Regional Burn Center.  In addition, Memorial is a center known for general surgery, orthopedic surgery cardiac services, critical care services, renal services, maternity services, psychiatric and rehabilitative services.

Clinical pharmacy services include: 
•    Rounding on critical care, internal medicine, trauma, transplant and bariatric surgery
•    Heart failure disease state management.
•    Unit-Based Medication Therapy Management
•    Antimicrobial stewardship
•    General Drug information
•    Pharmacokinetic dosing
•    Investigational drug services.

Memorial is the clinical practice site for three SIUE-School of Pharmacy faculty, with practices in critical care, internal medicine and infectious diseases. Pharmacists participate in many hospital committees and provide education to physicians and other health care professionals.

The Resident will be involved in hospital committees, completion of a residency project, presented at a regional conference, and provide continuing education for practicing pharmacists.  Residents will also have many opportunities to precept students.

Program Purpose:

Residents completing this program will have developed the necessary skills to provide direct patient care to patients in an acute healthcare facility and be able to provide smooth transitional care to an ambulatory or other environment. 

Desired Outcome:  Applicants who have completed Memorial’s PGY-1 program will have the necessary skills to provide direct patient care.

In collaboration with the SIUE- SOP, the pharmacy resident will have the opportunity to earn a teaching certificate through a hybrid, web-based learning platform.  Self-directed learning modules guide the resident through important concepts in preparation for delivery of formal didactic lectures, small-group facilitation, Precepting of students and assessment.

Requirements for Completion:

Upon successful completion of all program requirements and compliance with all conditions of the residency program, the resident shall be awarded a certificate of completion. Certificates will only be awarded to residents who have successfully completed the following:
•    Completed 85% or more of their Goals as achieved for residency (ACHR) and all other goals and objectives marked as “satisfactory progress” or “achieved” in PharmAcademic.
•    Satisfactory completion of all assignments by preceptors and RPD
•    Provide a one hour Continuing Education Program for practicing pharmacists
•    Present at the ASHP Midyear Clinical Meeting or approved equivalent
•    Prepare a Medication Use Evaluation (MUE) or equivalent
•    Provide an analysis of adverse drug events (ADE’s)

PGY1 Residency 

Mount Sinai Hospital’s PGY1 Pharmacy Residency program is a broad, comprehensive pharmacy practice experience that prepares residents for multiple professional challenges and career opportunities. Brief Overview of Program.
Our Pharmacy Department provides evidence-based, cost-effective and safe drug therapy, targeting optimal patient outcomes; diseases that impact the community (heart failure, diabetes, COPD and asthma) are a focus of the general medicine, transitions of care, and ambulatory care rotations. The PGY1 pharmacy resident can gain experience in areas that include general medicine, critical care, emergency medicine, pediatrics/NICU, oncology, infectious diseases ambulatory care, informatics, and pharmacy services management.   The faculty includes six pharmacy specialists in the areas of MICU, SICU, Oncology, Pediatrics and Emergency Department (2);  additionally, experienced general medicine pharmacists and managers are on staff.  Expectations of the resident include formal presentations to pharmacists, physicians, medical residents and nurses; authoring pharmacy newsletter articles; conducting MUEs; mentorship of pharmacy students and development and presentation of a major project.

PGY2 Health System Pharmacy Administration Residency 

Mount Sinai Hospital’s PGY2 Health System Pharmacy Administration (HSPA) residency provides hand-on, pharmacy practice experience in multiple areas of pharmacy management; one position is offered.  Currently, the residency is in its 3rd year.   Residents have benefitted from exposure to a wide scope of areas:  this PGY2 program has served as a great platform for launching a career in health care administration. Previous residents have obtained positions such as pharmacy operations manager and outpatient pharmacy manager. There is a need for the development of strong, future leaders and managers in health system pharmacy. The vision of Mount Sinai Hospital’s PGY2 HSPA program is to provide a wide range of experiences that will prepare its residents for a future role as a director of pharmacy.

Brief Overview of Program
A strong pharmacy operations rotation exposes the residents to scheduling, personnel management, human resources, information technology automation, 340B drug discount program, budget/finance, and much more. The PSJMC PGY-1 Pharmacy Residency Program is a 12 month comprehensive program designed to develop the entry-level practitioner into a clinician who can excel in the provision of pharmaceutical care to multiple patient populations. The resident will be provided with advanced training and learning experiences in pharmacy practice that build upon those obtained throughout their pharmacy career. Rush is a not-for-profit academic medical center located just west of downtown Chicago in the Illinois Medical District. Rush University Medical Center encompasses a 664-bed hospital serving adults and children, including the Johnston R Bowman Health Center, which provides medical and rehabilitative care to older adults and people with short- and long-term disabilities. We are currently building a brand new patient care tower with a state of the art emergency room that is all scheduled to open in January of 2012. St. Joseph Campus has a de-centralized practice model with pharmacists strategically placed in critical care areas (NICU, ED, ICU) as well as the outpatient oncology, infectious diseases, general medicine and labor & delivery units. All pharmacists are ACLS certified and participate in medical emergencies. The site is an affiliated medical education teaching site for the Medical College of Wisconsin. The Pharmacy Department trains IPPE and APPE students from a variety of pharmacy schools, including the University of Wisconsin Midwestern University, Concordia University Wisconsin, and Rosalind Franklin University.

Description/Goals of Residency Program:
The pharmacy residency program is designed to develop self-directed practitioners who are prepared to assume advanced practice in an acute care environment, eligible for board certification, and eligible for postgraduate year two (PGY2) pharmacy residency training.  The resident will acquire or enhance current pharmaceutical care, leadership and administrative skills.  Residents will function as effective members of the health care team with the ultimate goal of providing optimal pharmaceutical care to patients. Residents will demonstrate time management and independent thinking skills that will make them leaders within their practice settings and within the profession.

Required rotations include: Critical Care, Neonatal ICU or Women's Health, Emergency Medicine, Internal Medicine, Pharmacy Administration, Infectious Diseases, and Ambulatory Care (Anemia Management Clinic and Medication Therapy Management). Additional Ambulatory Care programs are being developed.   Elective rotations include Medication Safety, Cardiology, Geriatric Medicine, Oncology, Internal Medicine II, and Academia through Concordia University Wisconsin School of Pharmacy. Residents are expected to become ACLS certified and attend each cardiac/respiratory arrest while onsite. Residents are expected to complete a research project that improves the quality of pharmacy services or initiates a new pharmacy service.   Additionally, residents will serve as site preceptors for IPPE students and assist in the training of APPE students. A series of leadership discussions are held throughout the year by the North Market Director of Pharmacy Services and Pharmacy Clinical Manager.
